rehabilitation therapyIntroductionThe Manistee Medical Care Facility staff welcomes the opportunity to work with you. We are committed to offering the programs and services you need to allow for the best possible recovery. We trust that by providing you with information about rehabilitation, you can take an active role in your recovery. In addition to general information on rehabilitation you will receive at our facility, please contact us to see our outcome data on conditions we treat. It is our goal to exceed your expectations during your rehabilitation stay. We are committed to providing you with the best possible care. servicesRehabilitation services are provided at the MCMCF through Agility Health, one of the top 10 rehabilitation service organizations in the United States. Working closely with the staff, it is their responsibility to preserve your rights for quality of life. While we recognize that not everyone may completely restore function, it is the therapist's responsibility to identify each resident's skill, needs and opportunities and to design and implement care plans to maximize your strengths and abilities and prevent decline. The role of the therapy department is to focus on your abilities, interests, learning potential, functional needs, and realistic outcomes. Each therapist will provide treatment to you in the fullest capacity involving all inter-disciplinary team members. Appropriate restorative programs can help assure that you have not just gained the ability to do something, but you are able to perform at that level (stability) on an ongoing basis. The rehabilitation team is made up of occupational therapists, physical therapists and speech therapists. Partnered with the facility team, your success is our goal. Your stay with usWhat will rehab be like?On your first day at the facility, you will be evaluated by the rehab team to determine your needs. Your evaluation will consist of reviewing your medical records, your prior level of function, you medications, previous tests or rehab you have been involved in, precautions and your social history. The evaluation will continue with a series of tests involving your strength/walking tolerance, balance, ability to dress, bathe and groom, vision, hearing, coordination, communicating levels, cueing levels, cognition, precept ion, sensation, and use of any adaptive equipment.Once your evaluation is completed, you and your therapist will design a program that will provide you with the best rehabilitation for independence. You and your family will be provided with education regarding your therapy needs all along the way. A typical day in the facility will start with getting up and reestablishing your morning routines--having breakfast and getting dressed. You will have assistance in these tasks up to the point where your independence starts to develop. Once up, its time to get to the therapy gym. The treatment will consist of therapeutic exercises and activities based on your needs and promote your recovery. Team and Resident Care ConferencesOnce your treatment is underway, the team at Manistee Medical Care Facility will get together to assess the effectiveness of your care plan. The team will be looking at your progress toward goals, discuss your discharge timeframe, and start to put together recommendations for ongoing intervention if needed once you are discharged. Your case manager will let you know when and how these conferences will take place to facilitate your participation and your family/caregiver interaction. During your resident care conference this will by your opportunity as well as your family/caregiver to ask questions and interact with the team outside of the treatment session. Resident and Family SatisfactionThe staff at MCMCF strive to meet the needs of the residents and their families. We want to ensure your satisfaction with the services you receive. If, at any time, ;you have a question or a concern, please ask to speak with your nurse or social worker on duty. The role of the nurse and social worker is to advocate for patients needs. If further intervention is require, the Administrator and Director of Nursing will be happy to assist you. Remember, your feedback is very important to us. We want you to have confidence that if you bring up a concern or question, it will give us an opportunity to make your rehab stay more comfortable.
